Workers Permits
Students who are under the age of 16 may be required to complete a workers permit for employment.
Please download the Certificate to Employ, choose school year employment or summer employment, and fill out accordingly:
- Section A needs to be completed by the student and needs to have a parent/guardian signature.
- Section B needs to be completed by the Employer.
Once Section A and Section B are completed forms can be returned to the designated school representative. Section C will be completed by the school and sent to the Division of Labor Standards Office in Jefferson City.
Forms can be turned into Debbie Auer in the MHS Counseling office, or emailed to auerd@msdr9.org
